Clinical Definition
UARS represents a milder form of sleep-disordered breathing on the spectrum between primary snoring and obstructive sleep apnea. Key characteristics include:
- Increased Airway Resistance: Narrowed but not completely obstructed upper airway
- RERAs: Frequent respiratory effort-related arousals without significant desaturation
- Sleep Fragmentation: Disrupted sleep architecture due to frequent arousals
- Preserved Oxygen Saturation: Minimal or no oxygen desaturations during events
- Excessive Daytime Sleepiness: Despite low AHI, patients experience significant symptoms
UARS is characterized by a high RDI (>15) with a low AHI (<5), indicating that RERAs are the predominant respiratory event.

A normal RDI is < 5 events/hour, with minimal RERAs and stable respiratory patterns throughout sleep.

How It's Measured
UARS is diagnosed through in-lab polysomnography with nasal pressure monitoring and EEG to detect RERAs. Esophageal pressure monitoring may be used in some cases to measure respiratory effort more precisely.
Role in Diagnosis
UARS diagnosis requires demonstration of frequent RERAs on polysomnography in a patient with symptoms of sleep disruption. The condition explains excessive daytime sleepiness in patients with seemingly normal sleep studies based on AHI alone.
Role in Treatment
Treatment for UARS is similar to OSA and may include CPAP therapy, oral appliance therapy, positional therapy, or surgical interventions. The goal is to reduce respiratory effort and eliminate RERAs to consolidate sleep.
